Family Practitioner

Vancouver Coastal Health is seeking a 0.6 FTE family physician for the Primary Care/Stabilization Clinic.

The Primary Care clinic operates 12 hours per day, Monday through Friday and 8 hours per day weekends and holidays. The interdisciplinary clinic focuses on providing clinic based, outreach & satellite primary care to the most complex clients residing south of False Creek in Vancouver. A smaller group of clients retains a medical home at Raven Song and receives ongoing care there. To ensure access to this clinic, adults referred for care transition over time to more appropriate care settings once stabilized and connected to community supports.

Reporting to the Medical Coordinator and to the Medical Director, the physician who accepts this position will play a critical role in supporting the ongoing primary care needs and in the development of an inter-professional shared care team. He/she is responsible for providing direct clinical services to patients who present or are referred for their medical care. He/she fosters collaborative inter-professional practice environment that enhances client-centered services within the associated clinics/sites/programs. He/she works within a self-directed team of physicians and other inter-professional colleagues to achieve excellence in client-centered care within a complex multi-site environment. All clients are approached with a view to support the transition to less intense service settings as stability is gained.

A Medical Degree and eligibility for licensure, in good standing, with the College of Physicians and Surgeons of British Columbia. Current Canadian Medical Protection Association, or equivalent, coverage. Eligibility for a faculty appointment with the University of British Columbia, School of Medicine is required. Current methadone prescriber exemption is ideal or willing to or in the process of obtaining exemption.

In addition, a candidate interested in joining the Raven Song team will approach care by:
• providing team based care along with an interdisciplinary team (NP/MD/RN/SW).
• providing addiction services, assessments, supports, and opiate substitution.
• providing care to clients with multiple complex health care needs (i.e. HIV, Hep C, mental health/addictions) and psychosocial barriers (eg. Histories of trauma, behavioural issues).
• providing low barrier health care to youth.
• sharing in a rotation that includes week days, evenings and weekends and sharing responsibility for ensuring team physician availability during specific times of the year (eg. Rotation for working Christmas/Easter/summer holidays).
• providing patients with 24/7 access to services through rotation in an on-call service.
• providing outreach as needed within the interdisciplinary team.
• providing learning opportunities and support for students/residents.
• taking part in quality improvement initiatives to improve client outcomes. This includes projects across acute and community care which support service improvements for clients.
• Providing pro-active chronic disease management by utilizing EMR supports, PSP supports, QI initiatives, and maximizing use and scope of interdisciplinary team members.
• taking part in interdisciplinary team engagement events/opportunities.
• taking part in educational opportunities to advance practice and client outcomes.

Remuneration will be in accordance with the Doctors of BC/MSP negotiated clinical service contract (template available on the Doctors of BC website).
